The Jean Bishop Integrated Care Centre in Hull is an internationally-recognised example of transforming care for older people by providing convenient, personalised care and reducing unnecessary hospital admissions, enabling residents to keep fit, healthy and living independently in their own homes.
This neighbourhood approach provides a clinically-led team who primarily treat 12,000 frail older people in Hull with long–term conditions who are at risk of hospital admission.
The challenge
Built in 2018, the ICC convenes professionals from different services to form a multi–disciplinary, patient-centred team, including geriatricians, advanced nurse practitioners, GPs, pharmacists, therapists, social workers and the voluntary sector. The health and social care team are co-located alongside a full fire station, whose team provides falls prevention and response services to support with rehabilitation and recovery of patients.
The Solution
Sewell Advisory ensured the project moved from concept to a completed facility, including providing initial optioneering and a multi-faceted options appraisal, full business case in line with the HM Treasury Green Book, value engineering to ensure affordability, market testing of delivery to ensure value for money, and project management. We provided clear, accountable organisation of the scheme delivery, encouraging early stakeholder engagement with NHS and wider sector partners, removing barriers to access and ensuring the scheme aligned with wider public estate plans.
The result
Measured outcomes show that the Jean Bishop ICC has contributed to a 13.6% reduction in emergency hospital admissions for patients aged over 80, and a 17.6% reduction for patients in care homes, reducing pressure on acute facilities and increasing system efficiency, as well as improving individual patient outcomes.
