A world-leading integrated care centre

LOCATION: HULL   |   PROJECT AREA: STRATEGIC ESTATES ADVISORY   |   PROJECT STATUS:  COMPLETED

The Jean Bishop Integrated Care Centre in Hull is an internationally-recognised example of transforming care for older people by providing convenient, personalised care and reducing unnecessary hospital admissions, enabling residents to keep fit, healthy and living independently in their own homes.

This neighbourhood approach provides a clinically-led team who primarily treat 12,000 frail older people in Hull with longterm conditions who are at risk of hospital admission.

A multi-disciplinary team

The challenge

Built in 2018, the ICC convenes professionals from different services to form a multidisciplinary, patient-centred team, including geriatricians, advanced nurse practitioners, GPs, pharmacists, therapists, social workers and the voluntary sector. The health and social care team are co-located alongside a full fire station, whose team provides falls prevention and response services to support with rehabilitation and recovery of patients.
